Questions about insurance
Do you take Medicaid?
Yes. We take Florida Medicaid and several Medicaid managed care plans. Plans change, so call your office and we will confirm yours before your visit.
Can I switch my primary care provider to Caring Hearts?
Yes. Most Medicaid and Medicare Advantage plans let you change your primary care provider. Call your plan or call us, and we will help you make the switch.
Do I need a referral to see you?
No referral is needed to become a patient. If your plan is an HMO, you may need to choose us as your primary care provider first. We can help with that.
What if I don't have insurance?

What should I bring?
Your insurance card and a photo ID. If you are not sure which plan you have, bring the card and we will figure it out together.
