Care for high blood pressure, diabetes and cholesterol

These three often show up together, and none of them go away on their own. You need a provider who knows your history, checks in on a schedule, and adjusts your plan when life changes. That's what we do.

High blood pressure

High blood pressure usually has no symptoms. That's why it's dangerous. We check it at every visit, find the right medicine and dose, and help you make changes that fit how you actually live and eat.

Diabetes and prediabetes

We track your A1C, check your feet and kidneys, manage your medicines and refills, and tell you when it's time for an eye exam. If your numbers are close to the line, we'll help you stay on the right side of it.

High cholesterol

A simple blood test shows your numbers. If they're high, we'll talk through food, activity and whether medicine makes sense for you.

What a visit looks like

  1. 01We check your numbers. Blood pressure, weight and any labs that are due.
  2. 02We go over your medicines. What's working, what isn't, what needs a refill.
  3. 03We make a plan together. Clear next steps and a date for your next visit.

Care in your language, with food you actually eat

Advice like "eat less rice" doesn't help if nobody asks what you cook. Our team speaks Creole, French and Spanish, and we'll talk about real meals from your kitchen, not a generic handout.

Questions patients ask us

How often should I see a doctor for high blood pressure?

Often every 3 to 6 months once it's under control, and more often while we adjust your medicine. Your provider will set the schedule with you.

Can you refill my blood pressure or diabetes medicine?

Yes. Bring your current bottles or a list to your visit and we'll take it from there.

What's a normal blood pressure?

Under 120 over 80 is considered normal. 130 over 80 or higher is high blood pressure. Your provider will tell you what your target should be.

Do you take Medicare for diabetes care?

Yes. Medicare and most Medicare Advantage and Medicaid plans cover ongoing diabetes care. Call to confirm your plan.

Do I need a referral to see you?

No referral needed to become a patient. Some HMO plans ask you to choose us as your primary care provider first. We can help with that.
